Support Program
The support program provides supportive persons, place and activities to individuals with significant functional limitations to achieve and maintain community stability and independence in the most appropriate least restrictive environment. The supportive service focuses on teaching independent living skills for older adolescents and adults, allowing them to become successful and satisfied in their preferred environment.
An assigned mentor will provide weekly skills training and development to the client as needed. The program is prepared and skilled to work efficiently in diverse and transitional settings including foster care and residential group home placements. Working cohesively with the clients case workers and their supportive team participants, is beneficial in the rehabilitation progress. The client will participate in a therapeutic one on one alliance with our mentors to ensure that their rehabilitation plan provides a map for personal development, empowerment and community integration.

The Community & Homeless Outreach Program is open Monday through Friday from 9am – 5pm and 9am – 2pm on Saturdays
