Community Integration Program (For Adults)
OVERVIEW
Community integration provides support, growth and hope for adults suffering from persistents psychiatric disabilities. Based on an individualized assessment of the individual presenting for rehabilitation services, it is the intent to provide a program of skill development classes focused on improving the quality of life for individuals in their personal recovery process.
CIHS’s rehabilitation staffs will provide a combination of individual and group rehabilitation activities at the adult’s home and/or other appropriate community locations. Rehabilitation staffs will provide On Site (facility-based) and Off Site (community-based) services. Services provided are family focused, individualized and based on the adult’s and the family’s strengths and resilience.
CIHS will establish specific rehabilitation goals which target identified needs based on information from the referring mental health professional, a comprehensive assessment and meetings with the adult and his/her family.
COI SERVICES
Community Integration Assessment
The COI process begins upon admission to the program, trained rehabilitation, trained staff will meet face to face with the adult and his/her family, guardian to complete a functional skills assessment. The functional assessment is designed to identify each adult’s strengths, skills and needs.
Services Schedule (requires client’s participation):
- Both onsite and offsite services = minimum of 6 visits/services per month (2 onsite activities and 4 offsite visit
- Onsite OR offsite services (not both) = At least 6 activities/visits per month.

INDIVIDUAL & GROUP REHABILITATION ACTIVITIES
Group activities are designed to reduce client’s functional impairment through age- appropriate skills teaching. Clients will be given the opportunity to actually practice new skills both on-site and off-site. Clients will be taught the following skills:

Supported Employment Program
For most of our adult clients, a career is an essential part of their identity. Although much progress has been made in improving opportunities and access to employment; there’s a significantly higher rate of unemployment for people with mental health condition as compared to the rest of the population.
CIHS’s Supported Employment program provides eligible individuals with a variety of employment-related supports including:
- Resume development, interviewing skills and job search assistance.
- On-and off-site job coaching.
- Ongoing support and advocacy designed to assist individuals with attaining and maintaining their employment
ELIGIBILITY
- 18 – 64 years old
- Actively participating in mental health treatment
- Referred by a Licensed Mental Health Professional
- Diagnosed with a behavioral health diagnosis
- Willing to participate in COI services
- A Medical Assistance recipient
- Intake assessment with CIHS staff (this may take up to 2 hours)
REFERRAL PROCESS
1. Submit the following documents: Referral Form, Professional Assertion of Need (verification that adult meets clinical eligibility criteria) and a copy of the adult’s current Individual Treatment Plan.
2. COI intake coordinator will contact the family or guardian to schedule a face to face screening meeting to.
A. Assess the adult’s rehabilitation service needs and willingness to participate in COI services.
B. Determine the program’s ability to address the adult’s behavioral health needs.
PRIORITY POPULATION DEFINITION
- Inability to maintain consistent employment because of psychiatric symptoms
- Social behavior that results in interventions by the mental health system
- Inability, due to cognitive disorganization, to function in the community
- Severe inability to establish or maintain a personal support system
- Need for assistance with basic living skills

COI Program for adults is open Monday through Friday from 10 am – 2 pm and 9am – 5pm on Saturdays
