Homeless & Community Outreach
The Community & Homeless Outreach Program provides services to the community and Baltimore’s homeless population. Our services supports victims of domestic violence, substance abuse, veteran neglect and HIV/AIDS population suffering from psychiatric disability or who are chronically homeless.
Indviduals who qualify for our services will meet with our case managers to develop an individualized plan in an effort the barriers to housing and effects of psychiatric and effects of psychiatric disabilities.
Our case managers can assist with the following:
- Social Security
- Housing Assistance
- Emergency Shelter
- Supportive Services
- Government Entitlements
- Medical Assistance

The Commuity & Homeless Outreach Program is open Monday through Friday from 9 am -2 pm and 9 am -3 pm on Saturdays
