Our Process

Step 1: Referral
CIHS receives clients on a rolling basis. Typically, clients are referred by, friend, family, family member, school, hospital and community organizations. Clients can also refer themselves. Referral can be made through our website by clicking on “Send Referral”

Step 2: Intake & Screening
CIHS Intake Coordinator will contact referred client within 48 hours for a brief phone screening to assess their needs and willingness to participate in our Community Integration Programs. Clients will complete the phone screen and CIHS will recommend services as appropriate

Step 3: Therapist Collaboration
Once the phone screen has been completed, to move forward with the program, clients must commit to bi-monthly visits with a CIHS recommended counseling center.

Step 4: Initial Assessment
Clients will undergo an initial assessment to identify the individual’s strengths as well as the review of their physical and emotional health.

Step 5: Rehabilitation Plan
CIHS staff will perform a comprehensive assessment on each client and help them develop a rehabilitation plan. Each plan will be client center, reflect the client’s needs, personal goals and intervention strategies.

Step 6: Counselor Assignment
The fun begins when CIHS will assigns each client a counselor. The counselor will carry out the treatment plan reflected in the client’s rehabilitation plan by visiting with client at least 4x month (minor) or at least 6x month (adults). Visits are on-site (CIHS office) and off-site (home and community).

Step 7: Review
CIHS will review the client’s progress against their goal every 3 (minors) and 6 (adults) months. Likewise, CIHS will conduct in-person consultation with the client’s family and friends as necessary.
