Community Integration Program
Our Community Integration Program (COI) includes On-Site services available able at our Pikesville office. Off-site services are also available in the homes and communities served by CIHS.
Community Integration Program provides support, growth and hope for minors, adolescent and adults suffering with persistent psychiatric disabilities. Based on an individualized assessment of the person presenting for rehabilitation services it is the intent to provide a program of skill development classes focused on improving the quality of life for individuals in their personal recovery process.

The COI Program is open Monday through Friday from 9am – 5pm and 9am – 2pm on Saturdays
COI FOR ADUTLS
CIHS’s rehabilitation staffs will provide a combination of individual and group rehabilitation activities at the child’s home and/or other appropriate community locations.
Rehabilitation staffs will provide On Site (facility-based) and Off Site (community-based) services. Services provided are family focused, individualized and based on the child’s and the family’s strengths and resilience.
CIHS will establish specific rehabilitation goals which target identified needs based on information from the referring mental health professional, a comprehensive assessment and meetings with the adult and his/her family.
COI staff routinely evaluate progress with each adult and provide monthly summary reports to the primary therapist and other mental health professionals working with the child and his/her family.
COI SERVICES
Community Integration Assessment
The COIprocess begin upon admission to the program, trained rehabilitation, trained staff will meet face to face with the child and his/her family, guardian to complete a functional skills assessment. The functional assessment is designed to identify each child’s strengths, skills and needs.
On-site and Off-site Services
The child and his/her family or guardian choose the services they want to receive (onsite, offsite or both) and the location where they want to receive them. The services are provided based on a monthly calendar of combination of onsite and off-site activities. Monthly program calendars are provided to all children and their families a can been found HERE. Rehabilitation staff will schedule meetings with the child and their families/guardians based on the level of need and client preference and availability.
Services Schedule (requires client’s participation):
- Both onsite and offsite services = minimum of three visits/services per month (2 onsite activities and 1 offsite visit or 1 onsite activity and 2 offsite visits).
- Onsite OR offsite services (not both) = At least two activities/visits per month.

Individual & Group Rehabilitation Activities-
Group activities are designed to reduce client’s functional impairment through age- appropriate skills teaching. Clients will be given the opportunity to actually practice new skills both on-site and off-site. Clients will be taught the following skills:

Eligibility
- 5 – 17 years old
- Actively participating in mental health treatment
- Referred by a Licensed Mental Health Professional
- Diagnosed with a behavioral health diagnosis
- Willing to participate in COI services
- A Medical Assistance recipient
- Intake assessment with CIHS staff (this may take up to 2 hours)
- Submit the following documents: Referral Form, Professional Assertion of Need (verification that child meets clinical eligibility criteria) and a copy of the child’s current Individual Treatment Plan.
- COI intake coordinator will contact the family or guardian to schedule a face to face screening meeting to.
A. Assess the child’s rehabilitation service needs and willingness to participate in COI services and
B. Determine the program’s ability to address the child’s behavioral health needs.
Priority Population Definition
- Inability to maintain consistent employment because of psychiatric symptoms
- Social behavior that results in interventions by the mental health system
- Inability, due to cognitive disorganization, to function in the community
- Severe inability to establish or maintain a personal support system
- Need for assistance with basic living skills
